- Step 1 - Download and complete the Certificate of Origin. Once completed please email it to [email protected]
- Step 2 - We will contact you and verify your information and make arrangements for the $25 application fee.
- Step 3 - We will send you an electronic copy, as well as mail you the original copy to a physical address.
