If you would like the Chamber to process your Visa, please fill out your name and address on this form, provide two passport style photos of yourself, your passport and $145 in check made out to Citslinc International and send to the Chamber office at 611 Ocean St., Ste 1, Santa Cruz. If you need to mail your application, please use Fed Ex or UPS and enclose a pre-paid return slip. The visa application process takes approximately 20 days.
