Certificates of Origin
If you are exporting goods from California, you may need a Certificate of Origin. This form requires the authorized signature of the local Chamber of Commerce Secretary and the seal of the Chamber. You can use your company's own form, or download a printable Certificate of Origin Form and the Santa Cruz Area Chamber of Commerce can sign and seal it. Please contact the Chamber for pricing information.
Note: The rules for when a Certificate of Origin is needed change often. Please be sure to verify the current criteria requiring a Certificate of Origin before sending your shipment.
